Eligibility & Exclusions

Who is eligible?

Physicians or dentists under age 60, who are actively performing the duties of their profession at least 30 hours per week.  Applicants under age 50 can apply for up to $10,000 per month; members age 50-54 can apply for up to $6,000 per month and members ages 55-59 can apply for up to $3,000.

Benefits to age 70

Long Term Disability Benefits are payable up to age 67 for covered total disabilities beginning before age 55; to age 65 for total disabilities from ages 55 to 63; and for 24 months on and after age 65.  Coverage terminates at age 70.

You are considered totally disabled if during the first five years of a covered disability due to accident or sickness, you are unable to perform the substantial and material duties of your regular occupation (specialty), thereafter, your complete inability to perform the substantial and material duties of any gainful occupation for which you are reasonably qualified by training, education or experience.


Coverage will be renewed until the premium due date coinciding with or following your 70th birthday. Renewal will terminate for non-payment of premium, or the date you retire or cease working full time (30 hours per week) for reasons other than a disability for which you have been receiving benefits.

Waiver of Premium

If a covered disability occurs prior to age 60, beginning with the next premium due date after you have received benefits for six consecutive months, premiums will be waived and will continue to be waived for as long as you are receiving benefits.

Survivorship Death Benefit

If death occurs while total disability benefits have been paid for at least 12 successive months, an amount equal to  three times the monthly benefit amount for the month prior to death will be paid to the next of kin.

Choice of Waiting Periods

Choose the appropriate period of time for benefits to begin that’s most comfortable for you; 60, 90, or 180 days.  The longer the waiting period you choose, the more economical the coverage.

Premium changes with each age bracket. *Rates for age 60 and over are for renewal only.  Maximum benefit allowed at age 65 is $2,500 per month. Monthly benefits for total disability in excess of $2,500 will be reduced to $2,500. Premiums do not reduce. Insurance terminates at 70. 

Optional Partial Disability Benefit(Residual)

Partial benefit will be paid when as a result of a covered accident or sickness you are not totally disabled but incur an income loss of at least 20% of pre-disability earnings. The benefit paid will be based on the percentage of the loss of income. However during the first 6 months the minimum benefit will be 50% of your total disability benefit. This coverage ends at the age of 65.

New York Life reserves the right to change premiums on any due date only if they are changed for all the other insured’s in the same insurance class. For example, a “class” is a group of insureds who share the same age.  The changes can only be made once in any 365 day period.

Exclusions and Limitations

Preexisting condition is defined as an injury or sickness or any related condition for which a person consults a doctor, receives medical services, supplies, or takes medication during the 12 month period immediately before the effective date of insurance. 

Our policy does not cover any disabilities or losses caused or contributed to by: War or any act of war whether declared or undeclared; intentionally self-inflicted injury; traveling in, or flying any aircraft operated by or under the direction of any military (land, sea or air) authority or while in any aircraft being used for any test or experimental purpose; pregnancy beginning before or during the first 30 days after your policy takes effect; service in the armed forces of any country or authority (in such even the pro rata unearned premium will be returned to you); or commission or attempted commission of a felony. In addition, you will not be entitled to benefits for two periods of disability at the same time.

Benefits for disabilities due to mental, nervous disorders or to alcoholism, chemical dependency or substance abuse are subject to a maximum of two years of payment.
